Established in 1978, Elderwood provides skilled nursing care, specialized subacute care, rehabilitation, assisted living, independent living and memory care services in Western and Central New York as well as in Rhode Island and Pennsylvania. The company is owned by Post Acute Partners, which also operates the managed long term care plan Elderwood Health Plan, Elderwood Transportation and Woodmark Pharmacy, and has grown to become the largest network of rehabilitation, skilled nursing, assisted living and independent living communities in Western New York.
