Works in collaboration with the Community Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illnesses. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management. Will require a flexible schedule which includes working nights and weekends as needed when working with a community.

Highly Preferred:

  • Bilingual Spanish

Job Summary

Works in collaboration with the Continuum Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illnesses. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.

Bilingual in English and Spanish, Tigrinya, Arabic, or Russian, is preferred. Experience working in a community-based setting preferred, experience working in community events and organizing events is preferred. Knowledge of medical terminology is preferred.

Qualifications:

HS - High School Grad or Equivalent (Required) Driver's License - Certification - Other/National, Driver License - Certification - North Carolina Division of Motor Vehicles (NCDMV), Driver License - Certification - Virginia Department of Motor Vehicles (VDMV)
