Job Responsibilities:











Position Summary:

The Housing Supportive Services Care Coordinator provides care coordination, program development, and building oversight for individuals residing at a supportive housing site in collaboration with the Milwaukee County Housing Division and Property Management Company Cardinal Capital and/ Team Management.





Essential Duties and Responsibilities



  • Welcome new referrals and complete intake process, including all required documentation.
  • Meet regularly with individuals for scheduled and requested check-ins to support identified needs.
  • Assist individuals in developing Personal Recovery Crisis Prevention Plans and Individualized Service Plans in their own words.
  • Conduct wellness checks and meaningful interactions; act as a positive role model.
  • Provide medication reminders as needed.
  • Utilize Motivational Interviewing, trauma-informed care, and strength-based recovery approaches to promote engagement and recovery.
  • Coordinate care with the individuals care team.
  • Collaborate with Peer Support Specialists to meet individualized needs.
  • Schedule and participate in recovery-focused team meetings and crisis staffings to address changes in symptoms or behaviors.
  • Maintain strong working relationships with Milwaukee County, Cardinal Capital, Team Management and other community-based providers.
  • Schedule and participate in monthly staffings with key stakeholders to review progress and planning.
  • Develop and implement recovery-focused activity schedules for the site.
  • Create and manage staff and Peer Specialist schedules to ensure appropriate coverage and adherence to programming.
  • Facilitate educational and skill-building groups based on participant needs.
  • Monitor building maintenance needs, submit reports within 24 hours, and follow up to ensure completion.
  • Maintain accurate documentation of residence contacts, incidents, and services in hard and electronic files.
  • Ensure cares files comply with agency policy, government regulations, and applicable codes.
  • Report incidents promptly and notify On-Call Supervisor in case of emergencies.

Knowledge, Skills, and Abilities:


  • Ability to work effectively in a diverse setting with unhoused individuals experiencing mental health, substance use and/or co-occurring needs.
  • Strong communication skills; compassionate, patient, and able to maintain confidentiality in all communication.
  • Capable of working independently with minimal supervision while being a collaborative team member.
  • Skilled in multitasking, taking initiative, and applying knowledge across various situations.
  • Demonstrate commitment to the agency's mission and values.
  • Conduct self ethically and professionally in all interactions. Maintain respectful, collaborative relationships with staff, individuals served, and external partners.
  • Adhere to agency Code of Conduct, Core Values, and all relevant policies and procedures and

    funding source requirements.


  • Demonstrate cultural sensitivity and awareness of ethnic, gender, and disability-related issues.

Minimal Qualifications:


  • High School Diploma or equivalent.
  • 2 years of experience serving individuals with mental health, substance use, and/or co-occurring needs.

Working Conditions: Work is performed primarily in a supportive housing setting serving individuals with mental health, substance use, and/or co-occurring needs. Work schedule must be flexible to meet the needs of the people being served.

Physical Demands: Position involves movement within supportive housing sites. Able to navigate stairs and lift/carry 20 pounds.
