DaVita is seeking a Clinical Coordinator to lead dialysis care in a skilled nursing facility. In this role, you'll oversee patient care delivery, support a clinical team, and ensure treatment meets the highest standards of safety and quality.

Key Responsibilities:

  • Coordinate care for ESRD patients, including labs, vils, and outcomes

  • Lead and support a team of clinicians and PCTs

  • Deliver direct patient care in a skilled nursing facility

  • Monitor patient status and adjust care as needed

  • Ensure compliance with all clinical and regulatory standards

What You Can Expect:

  • Flexibility to float to other clinics and adjust to varied schedules

Qualifications:

  • RN license in the state of practice

  • CPR certification

  • At least 18 months of RN experience

  • Minimum 6 months of dialysis experience

  • Charge RN readiness approval required

  • CNN or CDN certification preferred

  • ICU, CCU, ER, or Med/Surg experience preferred

  • Supervisory experience preferred

  • Proficiency in MS Word and Outlook
