ChristianaCare is recruiting for a Medication Reconciliation Technician to work full time day/evening shift rotation. Every other weekend required; holidays as required.

In this position, you will collect current and accurate information from patients and families with respect to prescribed home medications for the patient. You will then enter the data into the electronic medical record (EMR) and produce an admission medication reconciliation sheet to be presented to a pharmacist for review, and subsequently made available for physician review.

Principal Duties and Responsibilities:
  • Collect information regarding all current home medications from patients or caregivers, primarily in the Emergency Department
  • Enter patient medication information into the EMR
  • Produce documents for review by pharmacist and subsequent physician review
  • Provide assistance to healthcare providers to facilitate the provision of medications
  • Navigate the ED tracking board to determine patient selection
  • Solicit other sources of information to verify accuracy of collected data
  • Determine time last dose of each medication was taken, medication compliance prior to admission, and recent antibiotic medications presecribed
  • Obtain medication allergy history and related reactions to the medication
  • Orient and train new pharmacy employees
Education and Experience Requirements:
  • High School diploma or equivalent required; Associate or Bachelors degree preferred
  • Previous hospital pharmacy experience preferred
  • Applicants enrolled in, or graduate of a 4-year bachelor program in the following health science majors are encouraged to apply and will be required PTCB certification in 1 year of employment: Applied Molecular Biology and Biotechnology Biology, Chemistry, Exercise Science, Health Behavior Science, Human Physiology, Medical Diagnostics, Medical Diagnostics Pre-PA, Medical Laboratory Science, Nutrition, Nutrition and Dietetics, Nutrition and Medical Sciences, Nursing, Occupational Therapy, Physical Therapy, Physician Assistant or Sports Health
  • Maryland Pharmacy Technician License required
  • Pharmacy Technician Certification Board certification (PTCB) required
  • Computer experience preferred (e.g., window-based programs)

Hourly Pay Range: $19.84 - $29.76 This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
