Sibley Outpatient Specialty Coordinator

Purpose of Job

Responsible for the coordination of Sibley Outpatient Specialty services, involving support to consultants and their staff, to support patient care coordination. Interacts with patients and providers to ensure efficient and effective delivery of care. Responsible for directing activities to provide optimum patient flow and minimize barriers in the provision of patient care using a systematic problem-solving approach.

Job Functions

1. 
   
   Acts as consultant and resource person to the Specialty clinical teams regarding patient care issues.
   
   
2. Organizes and implements a system for patient tracking and follow up activities to optimize continuity of care.
3. Participates in strategic planning activities as directed by clinic manager.
4. Solicits feedback from the Specialty clinical teams regarding operation of the department, monitors effectiveness and efficiency of clinical services.
5. Works in clinic and outpatient areas as needed.
6. Performs other duties as assigned.

Minimum Education/Work Experience

• Graduation from Medical Assisting Program, School of Practical Nursing
• Three years related clinical experience

Knowledge/Skills/Abilities

• Ability to meet and maintain the necessary background checks as aligned with position functions
• Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above

Preferred Qualifications

• Current LPN or RN Licensure or Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA)
• Current CPR certification
