UCLA Health is seeking a dedicated ICM Coordinator (Oncology) for a limited 6 month position to support our Oncology RN Case Managers in delivering high-quality, coordinated care to patients with advanced cancer. This role is central to ensuring timely referrals, authorizations, and care coordination services. By collaborating closely with nurses, providers, and patients, you'll help improve outcomes, reduce hospital readmissions, and enhance the overall patient experience.
Key Responsibilities
  • Coordinate and process urgent, routine, pre-service and retro authorization requests in line with UCLA Health and CMS guidelines.
  • Support Oncology RN Case Managers by performing health screening to identify patient risk factors, social barriers, and care gaps that require escalation.
  • Facilitate safe transitions of care between inpatient, skilled nursing facilities (SNF), and home settings.
  • Assist patients with access to services such as DME, transportation, mental health resources, and home health care.
  • Monitor high-risk patient hospital admissions, discharges, and follow-ups, ensuring continuity of care.
  • Develop non-clinical care plans with supervision from case management nurses.
  • Maintain accurate documentation in electronic medical records and prepare required reports.
  • Communicate effectively with patients, providers, vendors, and health plans regarding referrals and care coordination.
Note: Position may convert to Career.
Salary Range: $ 35.47 - $48.82 Hourly
Qualifications
  • Minimum of 2 years or more experience working in a Managed Care Case Management and care coordination environment preferred.
  • High school diploma, GED or equivalent.
  • Experience processing Inpatient and ambulatory Managed Care referrals.
  • Vodavi phone system experience preferred.
  • Ability to operate a wide variety of office equipment, including computers, printers, copy machines, facsimile receiver/transmitter, scanners and mailing equipment.
  • Proficient computer skills including working knowledge of Microsoft Excel and Word.
  • Ability to multi-task, work with frequent interruptions and meet deadlines.
  • Must be detailed oriented, attentive, organized, and able to follow directions.
  • Ability to communicate thoughts and information clearly and succinctly in writing as well as verbally.
  • Ability to lift up to 25 pounds.
