Outpatient Hospice Physician - .3 FTE

The Park Nicollet Hospice and Palliative Department is seeking a part-time (.30FTE) Hospice Physician to join its collaborative care team.

The Hospice Physician will provide direct patient care and symptom management to hospice patients while assuring care is in compliance with Medicare and other regulatory programs. The clinician will also serve as liaison between the primary physician and the hospice team, providing care and facilitating communication and continuity of the care plan. This person will function as a resource person on palliative care and the art of terminal care to the medical staff and other health professionals and volunteers. The clinician will also support the admission and care delivery process of hospice patients. Board certified or board eligible in hospice or palliative medicine is preferred but not required. Prior hospice experience highly desirable, with strong Family Medicine or Internal Medicine experience required.
