The Patient Navigator will be responsible for:

  • Working closely together with other members of the Boston Children's Hospital Accountable Care Organization (ACO) addressing social determinants of health for patient families across Martha Eliot's programs, including Primary Care, Adolescent Medicine, Social Work, Mental Health, and more.
  • Assessing patient resource needs, facilitating referrals to community partners and agencies, supporting families in obtaining discretionary and entitlement benefits and credit, and maintaining documentation of services provided in the electronic medical record and databases. Tracking and maintaining outcomes and supports strategic planning processes. Developing and maintaining data.
  • Completing in-depth nutritional assessments and housing referrals to partner agencies. Providing trauma informed care and resources to families.
  • Working with an interdisciplinary medical team and researchers to help ensure patient families have stable, healthy housing; are accessing available nutrition education and food resources; and are receiving all benefits and community resources available to them.
  • Working closely with hospital and community-based agencies and resources to maintain accurate account of resources available to families. Acting as a liaison with internal and community resources. Providing outreach and maintaining positive working relationships with providers/administrative staff. Reporting outcomes of referrals to referral source(s).
  • Attending and participating in care planning meetings as needed.

To qualify, you must have:

  • High school diploma/GED required. Bachelor's Degree in Human Services or a related field is preferred
  • Minimum of one year of closely-related experience required. Experience with the clinic population or work with community housing and/or nutrition resources is preferred
  • Ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations
  • Work requires the analytical skills to resolve problems that require the use of basic scientific, mathematical or technical principles
  • Bilingual Spanish fluency highly preferred
