Family Health Centers of San Diego (FHCSD) is passionate about providing exceptional health care to all, especially underserved communities with limited health care options. Family Health Centers of San Diego (FHCSD) is looking for a Case Manager II.Under limited supervision, performs duties providing intensive, therapeutic case management services to assigned individuals. Participates in, and supports, the planning, development, implementation, and evaluation of services in accordance with contractual and departmental requirements and guidelines. Specific assignments vary depending on program, grant, and patient demographics, with a focus on providing community-based healthcare services to the medically underserved. The Case Manager II offers intensive case management services to clients and conducts therapeutic interventions.

Responsibilities:

  • Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
  • Conducts initial and on-going assessment of client's health and/or support service needs. Sets level of client need.
  • Creates formalized case plans and goals with clients; clients are high-need and require more intense services.
  • Develops a written care-plan with the client, identifying problems and needs.
  • Document areas assessed, intended interventions, and expected results in measurable terms, with short and long term goals.
  • Updates plan as client's needs change.
  • Provides therapeutic interventions to clients.
  • Provides intensive individual support to high need clients.
  • Makes internal and community services referrals.
  • Identifies if more intensive support may be needed; include accompanying clients to housing services, appointments, social services, etc.
  • Focus on medical adherence, support, and appointments designed to maintaining necessary care.

Requirements:

  • 3 years of work experience providing human services to high risk, medically underserved, or relevant community health populations required.
  • Ability and means to travel as needed in a timely manner within San Diego County.
  • Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field required, or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements. Master's degree in relevant field preferred.
  • Ability to work well in both a team-based environment and independently.
  • Basic counseling skills, such as reflecting, active listening, and paraphrasing.
  • Bilingual in English/Spanish may be required depending on assignment.
  • Excellent written and verbal communication skills.
  • Knowledge of the community resources, health and social service systems in San Diego County and skill in establishing working relationships with community partners.

Our History

FHCSD is a 501(c)(3) nonprofit organization rich in history and tradition. In 1911, the San Diego Industrial School served as a settlement house that provided Barrio Logan's immigrant residents with dearly needed social services. When it was announced that the city was closing the building in October 1970, Laura Rodriguez and a group of community members attended a Barrio Logan Community Action Committee meeting where they passionately advocated that the location should continue to function as a community health clinic. When their request to continue services was rejected, the group occupied the building in protest.

After eight days of negotiations the City of San Diego and the community leaders reached a resolution. This led to the gifting of the building to the people of Barrio Logan and the establishment of a free clinic. Medical care delivery began two evenings a week, and the health center saw approximately 150 patients each month. FHCSD has since grown into one of the 10 largest Federally Qualified Health Centers in the nation. We're proud to honor our organizations heritage by continuing to increase access to quality health care while serving the community of San Diego.

$29.00 - $35.35

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
