The Behavioral Health (BH) Service Coordinator is responsible for performing service coordination activities within their scope of licensure for members with behavioral health, co-occurring behavioral and physical health, and/or substance use disorder needs. The BH Service Coordinator enables Cook Childrens Health Plan members with chronic conditions or complex needs to achieve an optimal level of health and independence through advocacy, assessment, planning, communication, education, resource management, care coordination, collaboration, and service facilitation.

Education:

  • Requires Masters of the Arts (MA) or Masters of the Sciences (MS) in Social Work and/or Behavioral/Mental health or related field.

  • Minimum of three to five (3-5) years direct care clinical experience in case management, service coordination, or population health with a broad range of knowledge regarding Serious Mental Illness (SMI) and Serious Emotional Disturbance (SED).

  • Knowledge of assisting with complex psychiatric/substance use disorder cases and discharge planning.

  • Service Coordination or Managed Care Organization experience preferred.

  • Experience in motivational interviewing techniques preferred.

  • Oral and written communication; Critical thinking; Organization and time management; Proficiency in Microsoft Office and internet/web navigation Proficiency in navigating Electronic Medical Record Customer service.

  • Ability to work independently & remotely upon orientation completion

Must exhibit strong skills in the following areas: Experience working with children, adolescents and adults who are diagnosed with behavioral health disorder and substance use disorders including but not limited to Bipolar Disorder, Schizophrenia, Major Depression, Borderline Personality Disorder, Oppositional Defiant Disorder, Anxiety Disorder and other related areas .

Certification/Licensure:

  • Current unrestricted State License required such as Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Master Social Work (LMSW) or Licensed Marriage and Family Therapist (LMFT).
