Job Details The Care Team Associate (CTA) provides support to the Nurse Manager and the infusion nursing staff. The CTA provides all data entry support functions for the team and is cross trained in the responsibilities of a Clinical Secretary, Health Information Assistant I and II and Receptionist. The CTA is responsible for creating an accurate and complete clinical record for all team admissions and discharges in the information system (Home Care Pathways). Information is gathered from worksheets, medication records, and verbal orders, and entered into the Home Care Pathways information system to generate Client Plan of Care (485). The CTA codes (ICD-9) both primary and secondary diagnoses. This position requires the ability to problem solve with strong attention to accuracy and detail. The CTA monitors the Recertification process and elicits the assistance of nursing staff to complete appropriate assessments and paperwork. The CTA enters and edits all OASIS documents. The CTA ensures clients are appropriately discharged from the Homecare Pathways system. The CTA reports to the Nurse Manager and receives direction from the Health Information Manager.Job Description * Provides secretarial assistance as assigned and appropriate.
