Department: Social Services Full Time 8:00am-4:30pm- Monday-Friday with rotating weekends & holidays GENERAL SUMMARY Leads the delivery of quality patient care and adherence to policies, procedures and case management protocols. Responsibilities: Actively participates in the process of monitoring, guiding and evaluating staff performance. Assists with coordinating activities, prioritizing workflow, and promoting decision-making and problem solving. Performs the duties of a Case Manager. Provides consultation for and/or management of patients requiring complex or advanced treatment/care. EDUCATION/EXPERIENCE REQUIRED Master's degree required 1 year of experience required.
