Summary

Assembles, analyzes and performs checks on medical records to ensure completeness according to standards of regulatory agencies and hospital policies. Assists physicians/therapists with record completion by educating them about standards and requirements. Prepares records for continued care appointments.

Essential Job Responsibilities

* Maintain departmental records and filing systems on a timely basis.

* Sort, open, collect and distribute departmental mail. Handle typing, network, and data entry as assigned.

* Analyze records to ensure completeness. Enter data to update chart status in computer system. Re-analyze records, indicating remaining deficiencies and re-route to appropriate physician/clinician for completion.

* Verify completeness of record according to regulatory standards. Prepare records for permanent storage.

* File/retrieve records to respond to requests for information.

* Copy patient information from open and closed files to respond to third party requests for information.

* Provide general assistance to physicians, therapists, hospital staff and patient/families regarding health information issues. Provide training to new health information staff and students/residents as assigned.

* Report record deficiencies for chart completion/reporting purposes. Interface with Medical Staff/professional staff and initiate process to ensure timely record completion.

* Perform chart audits and analyze information as requested.

* Maintain confidentiality of all patient information. Verify patient name, number and discharge date, making changes when necessary.

* Respond to phone and e-mail requests, complying with requests when appropriate or directing to proper staff for follow-up.

* Perform other duties as assigned by supervisor.

Customer Service Responsibilities

Demonstrate excellent customer service and standards of behaviors as well as encourages, coaches, and monitors the same in team members. This individual should consistently promote teamwork and direct communication with co-workers and deal discretely and sensitively with confidential information.

Responsibilities in Quality Improvement

Contribute by identifying problems and seeking solutions. Promote patient/family satisfaction where possible; participates in departmental efforts to monitor and report customer service.

Essential Job Qualifications

* Associate Degree or equivalent business classes/experience

* Database, spreadsheet and word processing skills

* Knowledge of medical terminology

* Previous medical office experience strongly preferred

* Knowledge of policies and procedures related to record content and regulatory requirements

* Outstanding organizational, interpersonal and communication skills required
