Summary

Triages incoming calls in an ambulatory setting from patients, makes clinical decisions based on protocols, and communicates with providers including physicians and mid-level providers. Provides telephone advice and decision support services. Identifies clinical escalation and provides effective resolutions.

Job Duties

  • Facilitates coordination of care across settings for a designated population ensuring that labs, studies, and referrals are completed and results are discussed with the patient.
  • Delegates care management tasks to clinical or care management staff in the primary or specialty office.
  • Responds promptly to incoming calls and follows protocols and workflows as directed by the practice.
  • Collects information regarding a callers/patients concerns, medical history, and description of symptoms via telephone.
  • Refills prescriptions for patients and places orders for appropriate testing and/or labs.
  • Determines urgency of the patient based on brief assessment and on familiarity with a patient's condition and history.
  • Performs a short evaluation of the patient's situation to estimate the severity of illness and obtains vital signs and mental status information.
  • Follows-up with patients on visits and/or abnormal results and refers patients to appropriate medical facilities or providers for care.
  • Refers those with high-risk chief complaints such as chest pain, abdominal pain, or severe headaches to the emergency department immediately or arranges for an ambulance.
  • Provides appropriate home health advice to those patients who do not need to go directly to the emergency department.

Minimum Qualifications
  • Technical School Diploma LPN program
  • 1 year
  • Ability to use electronic medical records to check patient history.
  • Excellent computer skills and emotional intelligence.
  • Must demonstrate time management skills.
  • Must demonstrate empathy for individuals seeking health care related information and assistance.
  • Must be assertive with a positive attitude and have the ability to learn a range of services.
  • American Heart Association Basic Life Support - State of Pennsylvania
  • LPN - Licensed Practical Nurse_PA - State of Pennsylvania

Physical Demands

Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
