Job Overview Under the supervision of the practices Supervisory Physician and local Practice Manager, the position will ensure that practice patients are seen in a timely manner by the appropriate clinician, and that appropriate coordination of care and communication is maintained with the patients and referring physicians. A primary goal of the role is to ensure exemplary customer service for patients and to triage appropriate clinical needs for patients as they arise. Job Description Minimum Qualifications: 1. Associates degree in Nursing. 2. Registered Nurse (RN) license 3. One (1) years of nursing experience. 4. Basic Life Support (BLS) certification. Hours: Friday Saturday and Sunday (12-8:30PM) Location: Malden Area Preferred Qualifications: 1. Bachelors degree in Nursing. 2. Experience in communications and customer service preferred Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned. 1. Assesses patients' medical problems and complaints by phone and in person in order to provide information and advice for home treatment, when indicated, and arranges for evaluation by a clinician when indicated, either by scheduling an appointment or telephone and/or electronic follow-up to practice clinicians 2. Educates patients and families by phone and in the practice about minor illnesses, medication usage, health maintenance needs, blood sugar monitoring, nebulizer usage and other prescribed health care regimens 3. Follow-up nursing visits (if RN) in collaboration with PCP for hypertension and diabetes management 4. Looks for INRs daily and communicates adjustments of anticoagulation in collaboration with PCP 5. Performs injections, diagnostic tests and procedures, as necessary 6. Collaborates with lead Medical Assistants (MA)/flow coordinator on MA technical competency 7. Routinely monitors patient flow during clinical sessions and consults with Team Lead and/or Practice Manager to make adjustments to the schedule accordingly to ensure optimal patient access 8. Communicates anticipated patient needs to appropriate physician or other clinicians 9. Triages requests for prescription renewals 10. Maintains familiarity with the activities and interests of the practice physicians and disease-specific programs in order to provide the most appropriate triage 11. Maintains familiarity and a close working relationship with the physicians and their clinical and support staff to facilitate timely and effective communication and scheduling 12. Promotes direct and timely access and problem solving for urgent clinical situations 13. Identifies and helps to resolve service delivery problems, as necessary, under the guidance of the Practice Manager, and recommends methods to improve and streamline services 14. Ensures exemplary customer service by providing consistent communication. Works appropriately within various managed care systems to make triage decisions. 15. Participates in practice's Quality initiatives, under the direction of the Medical Director and/or designated clinical representative 16. Patient contacts to be documented in EMR and forwarded when appropriate. 17. Shows respect for patients/family dignity and privacy in all patient/family interactions. Establishes and maintains mutual trust within the nurse-patient/family relationship. 18. Listens and acknowledges patients/family knowledge of self and engages their participation in care. Identifies cultural needs (i.e. language, etc.) to develop a care plan that enhances the health care experience. 19. Presents self in a professional and calm manner. Assumes responsibility and ownership for professional conduct. 21. Participates in lifelong learning activities that assist in developing and maintaining their continuing competence, enhance their professional practice and support achievement of their career goals. 22. Completes all education to remain current with registrations, licenses, certifications, and unit/hospital mandatory requirements as appropriate to position or role. 23. Incorporates new knowledge and strategies to initiate change in nursing care practices 24. Attends and actively participates in staff meetings. Accepts responsibility for reading staff meeting minutes when not able to attend.
