Greater Lakes PACT program focuses on individual's recovery, safety in the community, the promotion of wellness and independence of the individual in their residential structured setting, and the re-establishment to a higher level of social life functioning. These services are designed to support and rehabilitate individuals who are experiencing symptoms of severe mental illness that impact their ability to live in the community. Seventy-five percent or more of services are provided outside of the program office in locations that are comfortable and convenient for clients. The RN provides psychiatric and nursing services to clients enrolled in PACT, including medication administration and monitoring as well as general nursing care and client education. Collaborates with internal and external medication prescribers and other clinical staff to ensure effective client care. Most services are provided in the community via outreach. Responsible for rotating on-call shifts as assigned. The Registered Nurse promotes the Mission of GLMH with a style that reflects the culture, the values and the mission of the organization Essential Job Responsibilities: Monitors physical and mental status and provide nursing services to clients enrolled in PACT based on the client's expressed needs and treatment goals.
Provides ongoing administration and monitoring of medication for clients, following established protocols.
Collaborates with all other treatment providers, internally and externally, to help assure effective client care. This may include prescribers, pharmacies, primary medical providers, and families of clients, along with PACT clients and staff.
Assists other PACT staff with general client care management throughout the shift as needed, including participating in crisis intervention and management of client emergencies.
Completes all clinical and administrative paperwork in a timely and appropriate manner.
Position Summary We're seeking an RN PACT to join our incredible MultiCare Behavioral Health team. PACT (Program of Assertive Community Treatment) is a mental health treatment program that provides community-based, comprehensive, and individualized services. PACT services are provided in a collaborative, multidisciplinary, ongoing fashion to assist people with severe and persistent mental illness who have not been successful within a traditional outpatient treatment model.It's an exciting time to be a nurse at MultiCare. Pay and Benefit Expectations We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $48.87 - $86.89 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
