Job Summary: The Supervisor-TCM Care at Home (CaH) works at the direction of the AVP and collaboratively with department nursing leadership team to provide administrative, human resource and financial management for designated area of responsibility at all campuses. Functions in a Magnet Recognized large tertiary academic medical center with multiple hospitals licensed under Loma Linda University Medical Center (LLUMC). Exhibits knowledge and understanding of industry trends, customer needs, expansion opportunities, national benchmarks, public reporting, and pay-for-performance initiatives. Assists with making solid business decisions and process improvements with the goals of program growth and development that defines, focuses, integrates and coordinates the full continuum of care with improved patient and family experience. Pairs patients being discharged from the hospital with outside vendors who will provide continuum of care at home. Develops and maintains a collaborative, credible and trusting working relationship with internal departments and staff to build a large referral base. Participates in research and evaluation of contracting opportunities in new/renewing vendors for CaH. Sets clear expectations with vendors to align them with the mission, vision and values of the organization. Ensures quantity and quality of staff. Mentors and coaches staff in transforming culture, processes and clinical practice that meets the demands of cutting-edge health care delivery and nursing excellence. Develops skills that contribute to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed. Education and Experience: Bachelor's Degree in Nursing required. Two years of clinical nursing experience in an acute care hospital setting required. One year of progressive leadership experience which has provided opportunity to demonstrate leadership, administration, education and/or staff development preferred. Two years of case management experience preferred. Knowledge and Skills: Substantial knowledge of state and federal healthcare regulations and accreditation, finance, performance improvement, quality, patient safety, evidenced-based clinical practice standards, medical staff structure, legal aspects of care, and nursing scope of practice required. Demonstrates professional competency and ability to develop and champion department-specific clinical outcome. Ability to work effectively with a wide variety of staff. Able to read; write and speak with professional quality; use computer and software programs necessary to the position (e.g., Word, Excel, Power Point, Access); operate/troubleshoot basic office equipment required for the position. Able to relate and communicate positively, effectively, and professionally with others; provide leadership; be assertive and consistent in enforcing policies ensuring compliance with regulatory standards, rules and laws; think critically; use sound judgment based on factual information and clinical knowledge; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently; perform basic math and statistical functions; manage multiple assignments; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone, and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position. Licensures and Certifications: Active California Registered Nurse (RN) license required. Basic Life Support (BLS) certification issued by the American Heart Association required.
