Overview: Telehealth Navigator - Community Supports

The Telehealth Navigator will play a crucial role in supporting individuals participating in community support programs by facilitating their access to telehealth services. This position involves working directly within the community to assist clients in completing their Individualized Assessment and Treatment Plans (IATP) in collaboration with clinicians through telehealth sessions. The ideal candidate will have a strong commitment to enhancing access to behavioral health services and empowering individuals to achieve their wellness goals.

Core Responsibilities:

  • Serve as a point of contact for clients participating in community support programs, providing guidance and support in accessing telehealth services.
  • Assist clients in completing their IATPs with clinicians through scheduled telehealth sessions, ensuring they understand the process and goals.
  • Educate clients about telehealth technology, helping them navigate any technical challenges to ensure successful participation in virtual sessions.
  • Collaborate with clinical teams to ensure continuity of care and facilitate communication between clients and providers.
  • Conduct outreach in the community to identify individuals in need of telehealth support, fostering relationships with local organizations and service providers.
  • Maintain accurate records of client interactions, progress, and outcomes to inform ongoing support and service improvements.
  • Advocate for clients by connecting them to additional community resources and support services as needed.
  • Stay informed about best practices in telehealth and community support services to enhance client experiences.

Knowledge, Skills and Abilities:

    • Must be able to work collaboratively with multiple programs and multidisciplinary team
    • Ability to tolerate ambiguity, uncertainty, and change;
    • Ability to navigate electronic health records;
    • Ability to complete assigned duties while managing own schedule;
    • knowledge of serious mental illnesses (SMI) and co-occurring chronic health conditions
    • Must have the ability to lift up to 15 pounds on a regular basis

Education and/or Licensure Requirements:

  1. A high school diploma or equivalent.
  2. Minimum of 21 years of age.
  3. A valid driver's license, reliable means of transportation, and proof of automobile insurance coverage are required.

Salary Range: $47,840 - $56,190 per year, dependent on education and experience
