We are searching for a talented Community Health Worker II

Hours: 40hrs

Shift: Day shift; 8:00am - 4:30pm

Location: Southcoast Business Center - Fairhaven, MA; Position will follow a hybrid schedule

Travel: Driving on average 3 or more times per week required

**Available to regular status employees who are scheduled to work a minimum of 24 hours.

Compensation: Pay rate will be determined based on level of experience.



Responsibilities

Position reports to the Director, Manager, Team Lead or designee, in collaboration with a multidisciplinary care team, provide culturally appropriate health education, coaching, support, information and outreach in a variety of settings including community-based, primary care, ambulatory and acute care settings. Identify high risk patients utilizing effective listening and communication skills to address health and social needs. Utilize critical thinking to navigate systems and facilitate the coordination of care and community resources to meet the patients plan of care. Assist patients to practice healthy behaviors and access necessary social, medical, and behavioral health support services and programs. Provide culturally appropriate health education, and outreach in a variety of community and health care settings. Implement culturally tailored interventions to meet patients short-term and long-term health and social needs.



Qualifications

  • Equal to completion of 4 years of high school plus CHW certification and two additional specialized courses or certifications.
  • Three or more years direct experience in community-based outreach.
  • Proven ability for independent decision making and problem solving while performing effectively as part of a multi-disciplinary team.
  • Solid knowledge of community resources and effective teaching/coaching skills.
  • Must be CHW certified and have two job-related certifications including but not limited to: Alcohol and Drug, SHINE/CAC or SHIP certified, Peer Recovery Coach, Health/Life Coach, Mental Health Counselling, Behavioral Health, Integrative Behavioral Health, Licensed Addiction Counsellor, National Certified Addiction Counsellor Level I and II Certifications or Interpreter Certification(s).
  • Verifiable good driving record and reliable transportation.
  • Agree to maintain all certification requirements while in the position.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
