**Available to regular status employees who are scheduled to work a minimum of 24 hours. Southcoast Health is an Equal Opportunity Employer. Compensation: Pay rate will be determined based on level of experience. Responsibilities Position reports to the Director, Manager, Team Lead or designee, in collaboration with a multidisciplinary care team, provide culturally appropriate health education, coaching, support, information and outreach in a variety of settings including community-based, primary care, ambulatory and acute care settings. Identify high risk patients utilizing effective listening and communication skills to address health and social needs. Utilize critical thinking to navigate systems and facilitate the coordination of care and community resources to meet the patients plan of care. Assist patients to practice healthy behaviors and access necessary social, medical, and behavioral health support services and programs. Provide culturally appropriate health education, and outreach in a variety of community and health care settings. Implement culturally tailored interventions to meet patients short-term and long-term health and social needs. Qualifications
