Summary:

Responsible for the coordination of the Non-Texas Resident case management program, inclusive of un-funded and under funded patients who are not residents of Texas or reside within the state illegally as residents. Facilitates planning, organizing, implementation and management of all options and services available for non-Texas residents who are admitted for inpatient hospitalization in the Cook Childrens Medical Center and/or non-Texas residents identified within Cook Childrens Physician Network. This position will assess, plan, coordinate, implement and monitor options and services available for non-Texas residents who are admitted for inpatient hospitalization in the Cook Childrens Medical Center and/or non-Texas residents identified within Cook Childrens Physician Network. A complete assessment of all non-Texas residents will be completed by this case manager in order to insure: 1) quality of patient care; (2) cost effective utilization and outcome management; (3) communication between patients, families and members of the health care team; (4) expeditious movement through appropriate levels of care; (5) information gathering for QA & Risk Management by utilizing Joint Commission Accreditation of Healthcare standards and clinical indicators. Once the current hospitalization has been completed and the child is in no immediate distress, appropriate arrangements will be made to determine where appropriate continued medical care will be provided.

Education:

  • BSN preferred minimum of AD in nursing required

  • Five years clinical experience.

  • Bi-lingual desirable.

  • Three years experience in case management/utilization review required.

  • Highly desirable to have experience in quality assurance, discharge planning, clinical pathways and continuous quality improvement in a managed care environment.

  • An RN with exceptional experience, e.g. Pediatric or 5-6 years in highly desirable areas listed above would also be considered.

  • Computer literate.

Certification/Licensure:

  • Current Texas RN license.

  • CCM desirable, but not necessary.
