Minimum to Midpoint Pay Rate:$40.86 - $40.86 / hour

SummaryAssesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs throughout the continuum in various settings through communication of available resources to promote quality cost-effective outcomes. Provides discharge planning through assessment, referral and interdisciplinary planning for high risk population and also links people to systems that provide them with resources, services and opportunities to achieve and maintain maximum functioning. Requirements

Education: Associates Degree in nursing with ongoing efforts to obtain Bachelors Degree.

Experience: Minimum of 3 years clinical experience. Previous experience in discharge planning, utilization management, knowledge of community resources, assessment skills, and clinical and social factors that affect patients functional status at discharge preferred.

License: Currently licensed as an RN in the State of Florida.

Certifications: Case Management certification in: CCM, ACM, NACCM, or CMCN required within 5 yrs. Of employment.

Other: Communication and interpersonal skills, basic computer, analytical, grammar and spelling skills. Ability to effectively multi-task and prioritize responsibilities.
