We are looking for Crisis Case Manager to provide overall supports and monitoring to individuals and staff working in the ASD Crisis Support Home in Warner Robins. The case manager will provide case management services to residents in the program. Must be willing to work hours that are varied, based on the needs of the individuals and crisis home, and as directed by supervisor.

Job Responsibilities:

  • Provide all aspects of case management including, but not limited to, facilitating referrals, coordinating intake and discharge, follow-ups with stakeholders, etc.
  • Serve as a member of the interdisciplinary team providing crisis supports to individuals.
  • Provide guidance and supervision to direct care staff through daily visits to the crisis home (and IIH site, where applicable).
  • Conduct regular fidelity checks across all shifts and provide necessary feedback and training based on results.
  • Maintain accurate daily documentation of services.
  • Maintain system for collecting program specific information including key data points and quality outcome data.
  • Seek input and maintain effective communication with IDT and supervisor.
  • Attend all agency, departmental, and individual specific meetings, as directed.
  • Develop and implement safety plans, as appropriate.
  • Consult with fully licensed staff on issues of danger to self and others.
  • Ensure that all applicable service and funding (e.g. SSI, Medicaid, Rep Payee, etc) are obtained for individuals in services.
  • Supervise and assist in transport of individuals in crisis.
  • Maintain individual confidentiality.
  • Must report any suspected abuse, neglect or exploitation according to Benchmark and DBHDD requirements.
  • Other duties as assigned.
  • Comply with all standards to assure the health and safety of all individuals.
  • Report to work as scheduled.

Qualifications:

Minimum:

  • Bachelor's degree in social work, applied psychology, marriage and family therapy or program primarily counseling in content.
  • Certification and ongoing training in crisis intervention curriculum.
  • Valid CPR and First Aid Certification
  • Valid Driver's License and auto insurance.
  • Must be computer literate
  • Must be experienced and competent in profession and maintain any applicable license, training and/or certifications.

Preferred:

  • Master's degree in social work, applied psychology, marriage and family therapy or program primarily counseling in content.
  • Current licensure, in good standing, as LAPC, LMSW or LAMFT
  • Minimum of two years' experience working with individuals with intellectual and/or developmental disabilities and/or mental illness who are in crisis.
  • Experience providing case management services to individuals with ASD and/or behavioral health diagnoses.
  • Working knowledge of the DBHDD service system
